During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on June 26, 2025 and completed on July 7, 2025 as a part of probation. The Department issued a Special Order on April 15, 2025 to place the licensee on probation for a period of twelve months; prohibit new admissions of all school aged children for a period of 30 days; require licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The on-site inspection began at 2:19 pm and ended at 4:48pm. The inspector reviewed compliance in the areas listed above. There were 128 children present and 14 staff including administrator. The inspector reviewed 5 staff records on site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 11, 2025 . A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment. Staff #3, Staff#4, who had been employed for over two weeks, did not have a central registry search requested.
Plan of Correction: Central registry searches have been requested for all staff.
Standard 8VAC20-780-240-B
Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

In Staff #2, Staff#3, Staff#4, records, there was no orientation documentation found and it was over 10 days of the date of assuming job responsibilities.
Plan of Correction: All staff have completed form. They had been oriented prior to first day and were missing the signed form.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
There multiple benches in the small gym with visibly cracked corners.
Plan of Correction: All damaged benches have been removed

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

At approximately 2:19 p.m. on 6/26/25, there were 27 children present in Classroom 2, but only 25 were marked present on the attendance record.
