Inspector Notes

An unannounced monitoring inspection was initiated on 01/13/2025 and completed on 01/21/2025 in response to a self-report received by the licensing office on 01/13/2025 relating to staffing and supervision. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 02/03/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. Staff #1 placed a hot beverage on a low table in classroom #1. Child #1, age 5 years, fell into the table causing the hot beverage to spill on the child. The child received burns with immediate blistering of the skin.
Plan of Correction: Staff have signed Hot Beverage Policy as of 01/13/25 reminding how to handle hot liquids for lunch and no hot beverages in class.
