Inspector Notes

An unannounced monitoring inspection was conducted from 11:00 am - 12:00 pm. There were 15 children in care being supervised by 2 staff. The children were observed participating in craft activities, structured center activities, and eating snack. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of five children's records it was determined that not all records were complete. Evidence: The records for child A (start date: 01/22/23) and child B (start date: 01/22/23) did not have documentation of proof of identity on file as required.
Plan of Correction: Administration will request proof of identity from the parents of child A and child B.
