Inspector Notes
A monitoring inspection was initiated on 7/22/2022 and concluded on 7/22/2022. There were 30 children present with six staff supervising children, two administrative staff and a cook. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records and five staff records were reviewed. The inspection started at 10:00 am and concluded at 1:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations

There are no annual training hours documented for Staff #1 (hire date 4/1/2020; #2 (hire date5/24/2021); #3 (hire date 7/28/2020); #4 (hire date 8/19/2010)
Evidence:
1. Located on the infant/toddler playground is a Little Tykes house that has a broken window. This could entangle clothing or snag skin.
2. The Little Tykes house located on the big playground has holes on the side that are accessible to the children that could snag skin.
3. The tricycles on the big playground ground are rusting and have plastic pieces broken that could snag skin.
4. The Little Tykes trike has a seat that is missing, the picnic table top is broken which could snag skin. The Little Tykes climber has a green slide that has cracks at the bottom.
5. The intake vent in the K-3 room has dust and dirt that is accessible to the children.
Evidence:
1. The infant/toddler playground had grass growing in the resilient surfacing. There was no surfacing under the slide.
Evidence:
1. The big playgrounds did not have a shady area for the children.
Evidence:
During the inspection the LI observed staff #3 take her class out side to the playground. Child #1 (3yrs old) was not allowed to play and was made to sit on the ground for approximately 10 minutes while the other children played.
