During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/20/2025. The on-site inspection began at 10:20 am and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and 11 staff. The inspector reviewed three children?s records and four staff records on-site.
This inspection included: document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/29/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-210-A
Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent.
Staff #2, who has been actively working at the center for the past 10 months and staff # 4, who has been actively working as a program leader for the past five months, have not fulfilled a high school program degree or the equivalent.
Plan of Correction: Administration will ensure that new employees have a high school diploma or equivalent.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
The four playgrounds have weeds growing in the mulch and on the play equipment.
The plastic on the handle bars on the circle bikes are broken.
The railroad ties beside the indoor play building are deteriorating and have a piece of rebar sticking up.
Plan of Correction: Weeds will be removed.
Circle bikes will be removed from the playground.
Railroad tie will be removed and replaced with new one.
Standard 8VAC20-780-330-B
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.
The fall zone on the large playground measured to be two feet when each piece of equipment required at least six feet of a fall zone.
Plan of Correction: Administration will ensure that playground equipment meets the required fall zone of 6 feet.
