REPEAT VIOLATION
Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent.
Staff #2, who has been actively working at the center for the past 16 months and staff #3, who has been actively working as a program leader for the past 11 months, have not fulfilled a high school program degree or the equivalent.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
There was a nail in the door in the infant room that could entangle clothing or snag skin. There was one mobile infant in that area.
There was a saucer/bouncer in the infant room that had the material in the seat was torn. Also, a piece of the saucer/bouncer had a ball that held the seat into place that was broken. The broken piece had tape around it.
The shelves in the older infant area have peeling paint. These shelves have toys on them that the children can access.
