Inspector Notes
An unannounced monitoring inspection was conducted on 8/3/22 from 10:20am - 12:40pm. During the inspection there were 86 children ages four months old through ten years old in care with 14 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for nine children and nine staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.
Violations
3Evidence:
1. There were two picnic tables located on the large playground that had a section where the wood was cracked and splintering.
2. Staff #10 (Program Director) confirmed the picnic tables were not in a safe and operable condition.
Evidence:
1. The written authorization for the medication (Ventolin inhaler) for child #10 was not signed by the parent.
2. Staff #10 (Program Director) confirmed that there was medication for child #10 did not have written authorization from the parent to be given.
Evidence:
1. The emergency drill log did not have documentation to indicate an emergency evacuation drill was completed for the months of April through July 2022.
2. Staff #10 Program Director) and staff #5 both confirmed that an emergency evacuation drill had been completed each, but that the information was not documented on the emergency drill log.
