Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/24/24. The on-site inspection began at 10:30am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 15 staff. The inspector reviewed 7 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/1/2024. A POC submitted after this date will not appear on the public website.
Violations
2Staff #3, employed for 1 month, did not have documentation of requesting a central registry and sex offender check from one state and no documentation of requesting a central registry check, criminal history check and sex offender check from a different state.
The center is required to ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
On the playground there was standing water in a rocker seesaw and on a tarp covering the sand box area.
On the playground there were three screw heads protruding from a plastic fence and the bench of a picnic table was cracked and had a jagged edge. The screw heads and the cracked bench posed a scratch or snag hazard.
In the 3-4 year old classroom there was a finger sized hole in the wall and the drywall was exposed.
