Inspector Notes
A self-reported incident regarding supervision and care and protection was initiated on February 26, 2024. Staff interviews, staff and child records were reviewed, and observation of the toddler playground was completed. After carefully interviewing all involved parties, information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
4Evidence:
1. On February 8, 2024, at approximately 5:30 pm, Staff 1 and 2 reported at the end of their shift Child 1 (two year and eleven months old) was left unattended on the playground unsupervised for approximately 10 minutes.
2. Based on staff interview, the center reported the incident to Child Protective Services (CPS) on 2/23/2024.
Evidence:
1. On February 8, 2024, Child 1 (two year, eleven-month-old child) was left alone and unsupervised on the center outdoor playground for approximately 10 minutes.
2. Staff 1 and Staff 2 acknowledged they did not stop at each threshold to
complete a face-to-name check sheet as required by the center policy and were unaware that they left the child outside.
Evidence:
1.On February 8, 2024, a group of 9 children in the two-year-old classroom were on the outdoor playground approximately 3:30 pm to 4:30 pm per daily activity schedule. Approximately 4:30 pm, Staff 1 and 2 lined up 8 children to enter the building, Neither Staff realized Child #1 was in a hidden area on the playground. Staff 3 found Child #1 alone outside and crying approximately 10 minutes after the class went inside.
2. Staff 1 and 2 acknowledged they did not use name and face recognition at each threshold entering in the classroom.
Stgaff will report to administration when a child/children disrupt the Face-to-Name process.
Staff will perform a playground sweep and check spts that children can hide.
Evidence:
1. The center?s policy regarding supervision and playground safety of children were not followed when a child was left unattended on the playground on February 8, 2024, for approximately 10 minutes.
2. Child 1 was left outside because Staff 1 and 2 acknowledged they did not stop at each threshold to complete the face-to-name check as required by the center policy.
3. Staff 1 and 2 reported the incident at the end of their shift to Staff 4, but not immediately after it occurred.
4. The parents of Child 1 were not notified that their child was left unattended on the on the outdoor playground until the following day.
Retrained Staff Member 1 and 2 invidividually on Face-to-Name procedure.
Retrained entire staff on Face-to-Name and Mandated Reporting. Staff has been trained that anyone can report to CPS and State Licensing, they don't have to wait for Principal of the school to do so.
