Inspector Notes

A monitoring inspection was initiated on April 18, 2023 and concluded on April 19, 2032. There were twenty-five children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, facility did not ensure hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children.

Evidence: Cleaning supplies were observed in an unlocked cabinet used by the program in the cafeteria. Staff stated the cabinet does not lock.

Based on observation and interview, facility did not ensure there is at least one staff available for every eighteen children when the youngest in the group is school age up to nine years old.

Evidence: On April 18, 2023, one staff was present in the gym with twenty-two school age children from the time of school dismissal (3:30 - 3:45) until 4:07 when the children were brought to the cafeteria. Staff confirmed having twenty-two children in care with no other staff present in the gym. The second staff on site was in the cafeteria with three children.
