Inspector Notes
An announced inspection was completed on 09/25/2023 in response to a relocation of the licensed program.
There were 25 children, ages 4 to 9 years old, present with 3 staff supervising during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed arriving to the program and playing gross motor games in the gymnasium. A total of 5 child records and 3 (all) staff records were reviewed. The inspection started at 1:15 pm and concluded at 4:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law. Any areas of noncompliance are documented in the violation notice.
Violations
2Evidence:
The program is operating at a new location, the newly constructed Hardy Elementary School, 9389 Old Stage Hwy. Smithfield VA 23430.
The current Child Day License issued to the program permits operation at 9311 Hardy Circle, Smithfield VA 23430.
The licensee did not inform the Department of Education of the program's relocation prior to operating at a new facility.
The Department became aware of the program's relocation during the process of obtaining a complete application for the renewal of their License to operate.
The department received written notification of the new location, after requesting it, on 9/12/202.
The program director stated that the program had been operating at the new location since 9/5/2023.
An initial licensing inspection for issuance of the new License at the new location was conducted on 9/25/2023.
The Licensee will give adequate notice, preferably 60 days in advance, of any changes to the current License to operate and will not initiate such changes until they are issued a license that reflects that the change has been approved.
Evidence:
Five children's records were reviewed.
The record for child #2 contains documentation stating that child #2 is allergic to eggs.
Child #2's record does not contain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
The program director confirmed that an allergy care plan had not been received for child #2.
