During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 11/6/2024 and completed on 11/12/2024. The on-site inspection began at 4:10 pm and ended at 5:50 pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records [off site at the business offices 0n 11/12/2024. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today.

3During the inspection, 3 eight-year-old children and 1 five-year-old child were present without staff for approximately 5 minutes int he boys bathroom at the opposite end of the hall from the multipurpose room where staff were located.

Staff #1, who has been working with children in the program for over 9 months, and is the designated Lead Teacher of the site, does not have documentation on record to demonstrate her qualifications.
