(Repeat Violation) Staff who work directly with children must, in addition to preservice and orientation training, annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.
Staff #3 has not completed Virginia Health and Safety Training, which was overdue by more than 4 months.
Staff #4 has not completed the Annual Virginia Health and Safety Training, which was overdue by more than 5 months.

Plan of Correction: It is possible that staff #3 and 4 have completed the Annual Health and Safety Update Training but did not turn in their certificates. Staff will be asked to turn in certificates if the training has been completed or to complete the course and turn in their certificates of completion within 30 days.
