Inspector Notes
A renewal inspection was conducted on March 6, 2024 from approximately 9:25 AM to 1:14 PM. There were 63 children present in the center, ranging in ages from infant to Pre-K, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records, six staff records and 2 board member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

3Evidence:
1. The record of child #3 (DOE: 6/01/23) contained an immunization record dated 5/12/23. The documentation of additional immunizations for child #5 (12 months) should be obtained once every six months for children under two years of age.
2. A member of management acknowledged that the immunization update was missing.
Evidence:
1. The record for staff #5 (last TB screening date: 1/21/22) contained a TB screening dated 1/30/24.
Evidence:
1. The record for M #4 (3 years of age) had a diagnosed food allergy but did not have an allergy action plan on file.
2. A member of management acknowledged that an allergy action plan was not in the record for M #3.
