Inspector Notes

The licensing inspector conducted an unannounced complaint inspection on February 21, 2024 from approximately 12:30 PM to 1:47 PM in response to a complaint that was received by the licensing department on February 20, 2024 ,regarding allegations of staff and supervision and behavioral guidance. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Based on a review of the facility's injury reports, all required information was not documented.

Evidence:
A total of eight injury reports for the month of February 2024 were reviewed; five reports did not have the date or time of parent notification, five reports di d not have how parent was notified, and four reports did not have staff and parent signature or two staff signatures.
