Inspector Notes

An unannounced inspection was initiated on June 18, 2024 and completed on June 18, 2024 in response to a complaint received by the licensing office on June 3, 2024 relating to allegations of supervision, forbidden action, and injury reporting. The inspector reviewed three staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to staff to child ratio and are documented on the violation notice issued to the program.

REPEAT VIOLATION
Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
The record for staff #3 contained an updated TB screening results of 1/30/2024. A TB screening was due on or before 1/21/2024.

Based on record review, the center did not maintain ratio 1:18 (staff: children) for school age eligible up to 9 years as required.

Evidence:
At approximately 3:35 PM the school age classroom had 23 children and one staff.
