During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
The inspection findings have been updated as a result of a first-step review completed on May 29, 2025.
An unannounced inspection was initiated on 4/22/2025 and completed on 4/22/2025 with a local agency, in response to a complaint received by the licensing office on 4/21/2025 relating to allegations of forbidden actions and behavioral guidance. The inspector reviewed one child?s records and three staff records on-site on 4/22/2025. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid.Though not a part of the original complaint, violations that were not reported were found related to discipline policies and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/9/2025. A POC submitted after this date will not appear on the public website.

Child #1 was held by the hand and around the wrist for 24 minutes by Staff #1, requiring the child to stay near to the staff?s side without being able to engage in any activities. Child #1 can be seen in video footage trying to pull away from Staff #1.

On April 16, 2025, Staff #1 held Child #1 by the hand and around the wrist for
approximately 24 minutes as a form of behavior guidance. Child #1 can be seen
on video footage trying to get free from Staff #1?s grasp.

Video footage reviewed, showed on 04/16/2025 during naptime Staff #1 roughly pulls Child #1 by the shirt to her at the table where she was sitting; Staff #1 can be seen pulling Child #1 towards her by the shirt and gets very close in Child # 1?s face and starts speaking; Child #1 is pulled down by his arm to a sitting position on the floor. Child #1 turns around and tries to move away from Staff #1. Staff #1 then grabs Child #1 by the arm and slid him across the floor towards her. Staff #1 held Child #1 by the arm, wrist and forearm, without release for 24 minutes. Child #1 can be seen trying to pull away from Staff #1.
Video footage reviewed, showed on 04/16/2025 during a transition time to outdoors, that Staff #1 pulled Child #1 by the hood to the back of the line where she was standing. Child #1 tries to get loose of Staff 1?s grasp. Staff #1 grabs on to Child #1 shirt and jacket and pulls him back and grabs his hand. Child #1 falls to the floor on his knees, and Staff #1 pulls Child#1 by the arm to a standing position.

The handbook provided by the center does not list unacceptable discipline policies. It was confirmed by Staff #5 that no other policies have been provided to the parents.
