Inspector Notes

An unannounced monitoring inspection was conducted on 04/03/2023. There were 168 children, ages 3 years-5 years, 24 staff providing direct care and supervision, and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed in the following activities: participating in a special outside activity, free choice play in centers, participating in learning activities and during lunch. A total of 10 children?s records, 10 staff records, and 3 medications were reviewed during the inspection. The inspector arrived for the inspection at 10:00 am and departed at 1:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observations, the center failed to ensure that restrooms shall be equipped with soap, toilet paper and disposable towels or an air dryer within reach of children.

Evidence: There were no disposable towels or an air dryer in the upstairs girls bathroom of the main building of the center.
Plan of Correction: Will supply paper towels in the bathroom.

Based on record review, the center failed to ensure that each staff record contains all of the elements as required by the standards.

Evidence: The record for Staff 7 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment; there were no reference checks available for review. Staff 7 was observed working during the inspection.
Plan of Correction: Will get reference checks.
