Inspector Notes

An unannounced, on-site monitoring inspection was completed on 10/09/2024. The on-site inspection began at 9:00 am and ended at 12:10 pm. The inspector reviewed compliance in the areas listed above. There were 141 children present, 18 staff providing direct care and supervision, and various support staff. The inspector reviewed 10 children?s records and 10 staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A tuberculosis (TB) screening is required every two years from the last screening.

The updated tb screening for Staff 4 was due 2 months ago. The updated screening has not yet been completed.
Plan of Correction: The staff was asked to get an updated screening during the inspection.
