Inspector Notes

An unannounced, on-site monitoring inspection was completed on 09/16/2025. The on-site inspection began at 9:05 am and ended at 12:55 pm. The inspector reviewed compliance in the areas listed above. There were 135 children present, 18 staff providing direct care and supervision, and various support staff. The inspector reviewed 10 children?s records and 10 staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances, such as cleaning materials, insecticides, and pesticides, shall be kept in a locked place using a safe locking method that prevents access by children.

The lock on the cream cabinet in Class 6 was not properly locked and could be opened by pulling the handle; there were various cleaning materials and an insecticide on the bottom shelf of the cabinet.
Plan of Correction: The cabinet was properly locked during the inspection.
Standard 8VAC20-780-280-E
Cleaning materials and insecticides/pesticides shall be stored in areas physically separate from each other.

There was a basket of cleaning materials that also contained an insecticide on the bottom shelf of the cream cabinet in Class 6.
Plan of Correction: The insecticide was removed during the inspection.
Standard 8VAC20-780-510-N
The center record of medication administration shall include all of the components required by the standards.

The medication administration log for Medication 4, administered daily, did not include the time the medication was administered as required by the standards.
Plan of Correction: Will begin to use the VDOE model form.
