Inspector Notes

An unannounced complaint investigation was initiated on December 5, 2023 from 4:15 p.m. to 5:30 p.m. in response to a concern received by the Valley Licensing Office on November 30, 2023 in the areas of programs, supervision and special care provisions and emergencies.

Observation of the program was conducted. Staff interviews were conducted and documentation was reviewed including accident and incident reports.

Additional interviews were conducted off site. Additional documentation was reviewed off site.

Based upon the preponderance of the evidence collected during the investigation, the complaint regarding forbidden action, behavioral guidance, care, protection and guidance, parental involvement and procedures for emergencies was determined not valid.
