Inspector Notes
An unannounced monitoring inspection was conducted on 5/31/23 from 12:30 a.m. ? 2:30 p.m. There were forty children present with five staff providing supervision. The ages of the children ranged from 4 to 5 years. Three staff files and four children?s files were reviewed. Children were observed playing outside, eating lunch and preparing for a birthday party. Handwashing was also observed. Attendance and emergency drills were reviewed. There was an adequate number of staff present with certification in Medication Administration Training (MAT), CPR, First Aid and Daily Health Observation (DHO) training. Three medications and their authorization forms were reviewed. Required postings were observed. The last emergency drill was conducted on 5/20/23. The first aid kit, flashlight, and battery operated radio were observed. Areas of non-compliance are identified in the Violation Notice.

6Evidence: On the date of inspection, the findings of the most recent inspection of the facility was dated 5/12/22. The most recent inspection report for the facility was 12/7/22.
Evidence: The record for Staff # 2 did not contain documentation of education required for the job position.
Evidence: The record for Staff #1, with a start date of 7/9/22, did not have documentation of
completing the Preservice orientation course on file.
Evidence:
1. In the downstairs classroom the unlocked electrical room, located in the classroom, had pesticides and cleaning agents with warning labels stating keep out of reach of children.
2. In the upstairs classroom a lotion, with a warning label stating keep out of reach of children, was located on the sink accessible to children.
3. In the upstairs classroom dish detergent, with a warning label stating keep out of reach of children, was located on the sink accessible to children.
Evidence: In the downstairs classroom there were three lunch boxes with no name.
Evidence: The records for Staff #1 and Staff #2 did not contain documentation of the person making the call to check references.
