Inspector Notes
Conducted an unannounced inspection at 11:00am. Observed 84 children + 15 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: art, writing even + odd numbers, spelling words, playing musical instruments, having outdoor active play, washing hands, eating lunch, and taking naps. Staff were actively engaged with the children and activities. Lunch served: beef ravioli and tomato sauce, salad, peaches, and milk. The facility was found to be clean and well supplied toys and equipment for the children. An area of non-compliance with standards and laws reviewed was found. Violations
12. The previous expired search was completed on 9/13/17, thus requiring a new search to be completed by 9/12/22.
On 1/9/23, the inspector received an email from center management requesting the following statement be added to the plan of correction: The report was sent on 11/28/22, prior to it's due date. The delay is with department of social services. LAP will again follow up to see why there is a delay in their processing.
On 1/11/23, the inspector received from center management a completed Central Registry Search for Staff #2, dated as completed on 12/19/22.
