Inspector Notes

Conducted an unannounced monitoring inspection at 10:00am. Observed 67 children + 16 direct-care staff. Ratios and supervision were in compliance. Today was the last day of school for the school-year program and the center was closing at 12:30pm. The center was busy with year-end cleaning and reorganizing to begin the summer camp program next week. Children were engaged in a variety of activities to include: having a pajama day, watching child appropriate shows/movie, eating snack, having story-time, playing with blocks and toys, having outdoor active play, and some of the older children were assisting staff with simple cleaning of toys. Staff were actively engaged with the children and activities. The center was found to be clean and well supplied toys and equipment for the children. Based on observations made, the licensee failed to ensure that resilient surfacing on the playground was in compliance with minimum safety standards. Evidence: There was less than 6" of wood mulch in the fall zones of equipment. The mulch depth was approximately 1"-2" and was scattered to expose the dirt and sand underneath in some areas.

Based on records reviewed, the licensee failed to ensure that a monthly emergency evacuation drill was documented. Evidence: There was no documentation of a drill being completed for January 2023.
Plan of Correction: Drill was conducted on 1/23/23 but forgot to document.
