Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a in-person tour of the program A subsidy inspection was initiated on (date) and concluded on (date). A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-580-E-1
Based on a review of three staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence
1. The record for staff #2 contained a cardiopulmonary resuscitation (CPR) certification that expired in June 2019.
2. The record for staff #2 contained a cardiopulmonary resuscitation (CPR) certification that expired in September 2021.
3. Staff #4 (Program Director) confirmed that staff #2 and staff 33 did not have a current certification in cardiopulmonary resuscitation (CPR) .
Plan of Correction: The facility responded: Both staff will be signed up to complete a cardiopulmonary resuscitation (CPR) certification course.
