Inspector Notes
A subsidy inspection was conducted on 4/20/22. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration.
Evidence:
1. The record for staff #4 did not contain documentation of a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
2. Staff #6 (Program Administrator) confirmed that staff #4 did not have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
