Inspector Notes
The licensing inspector conducted an unannounced complaint inspection on 5/1/23 in response to a complaint received in the licensing office on 4/24/23 regarding allegations in the areas of Supervision, Physical Plant and Special Services. Interviews were conducted with staff, records were reviewed and observations of the program were made relating to the allegation. Based on available information gathered during the inspection the complaint was determined to be valid; noncompliance(s) with the applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the area Director and the designated person in charge during the exit interview.

5Evidence:
1. When requested during the inspection, the most recent annual health department approval was not provided.
a. Staff #2 stated that everything they had was posted on the board, but the annual health approval was not posted.
Evidence:
The following conditions were observed in the bathrooms used by children:
1. There were 7 holes in the bathroom wall approximately 1/4-1/2? wide with insulation exposed and on the bathroom floor.
2.There was chipped and peeling paint in several bathroom stalls.
3.The stall door in the girls' bathroom had exposed screws with sharp tips.
4.The hinge on a stall door in the boy?s bathroom was loose by approximately ??. Screws were loose and protruding from the hinge and door.
Evidence:
1. Upon arrival the center director was not present in the center.
a. The staff on duty were not clear as to which staff member was in charge during the directors absence.
Evidence:
1. There were 6 cots in use that did not have the required linens.
a. In the preschool room 2 children did not have a bottom cover on the cot.
b. In the pre-k room 3 children did not have a bottom cover on the cot.
c. In the pre-k room 1 child did not have a top or bottom cover on the cot.
2. Staff #3 confirmed that several cots in use by children did not have either a top or bottom cover.
Evidence:
In the pre-k classroom there were 5 lunch boxes that had no dates on them, nor any names to identify the owner.
