A Subsidy Health and Safety Inspection (SHSI) was conducted on 10/25/2023 from 9:15am-11:15am. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

(Repeat violation)
Based on a review of ten staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. The record for staff #4 (date of hire 6/19/2023) and staff #10 (date of hire 7/16/2021) did not contain documentation of a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
2. Staff #2 (Regional Youth Director) confirmed that staff #4 and #6 did not have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Based on a review of ten staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Plan of Correction: The facility responded: Staff #4 and #10 is scheduled to attend, and will complete CPR/first aid on 10/26/2023.
