Inspector Notes

An unannounced monitoring inspection was conducted on 5/2/2024 from 12:00pm to 1:30pm. At the time of the inspection there were 21 children in care and 3 staff members. The facility tour included classrooms, bathrooms and the playground. Children were observed washing hands and transitioning to rest time. Five children's records and four staff records were reviewed. Evacuation drills, emergency supplies, medication and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on a review of documentation and interview, it was determined that the facility did not ensure that an annual approval from the health department shall be provided.
Evidence:
1. The health inspection was completed on 3/23/23, more than 12 months ago. An updated health inspection was due as of 3/24.2024.
2. Staff #5 confirmed that the annual inspection from the health department was not current.
