Inspector Notes

On 11/9/2022, an unannounced investigation was conducted regarding a self-reported incident by the facility regarding care and protection of children. Interviews were conducted, documents were reviewed, and observations were made. On 11/17/2022, video camera footage of the incident was also reviewed.
