During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on November 14, 2024. The on-site inspection began at 10:15am and ended at 1:15am. The inspector reviewed compliance in the areas listed above. There were 96 children present and 16 staff. The inspector reviewed 9 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and Measurements

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 26, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #1 TB was dated over four months prior to date of employment and has been employed for one month. Staff #2 TB was dated over five months prior to date of employment and has been employed for two months.
Plan of Correction: Staff members completed their TB tests while waiting for background checks to be cleared before they could start work. Going forward, I will have staff wait to complete the TB test until background check results have been received.

Repeat Violation

Areas and equipment of the center, outside, shall be maintained in a clean, safe and operable condition.

On the preschool playground, the large playground structure had a bolt sticking out and a corner piece missing from the climbing structure on the middle platform leading to the slide.
Plan of Correction: Work order will be submitted to correct the issue and that section of the playground will not be in use until it is corrected. Work order will be submitted by 11/22/24.
Standard 8VAC20-780-320-B
Each restroom area provided for children shall have sinks located near the toilets and that are supplied with running warm water that does not exceed 120 degrees Fahrenheit.

In the bathroom, used by 4-5 year old children, the warm water temperature was approximately 133 degrees Fahrenheit.
Plan of Correction: Work order will be submitted to have new anti-scald regulator attached to the sink.
Standard 8VAC20-780-500-B
The diapering area shall have a sink with running warm water not to exceed 120 degrees Fahrenheit.

The sink located in the diapering area, used to wash infants hands, was approximately 130 degrees Fahrenheit.
Plan of Correction: Work order will be submitted to have new anti-scald regulator attached to the sink.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #1, who has been enrolled for seven months, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: We will request that the parents provide an allergy care plan for the child or a note from the doctor that the allergy is no longer valid.
