During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on February 10, 2025, and completed on February 14, 2025, in response to a complaint received by the licensing office on February 7, 2025, relating to allegations of Staffing and Supervision and Programs. The inspector reviewed 3 children?s records and 4 staff records on-site on February 10, 2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 4, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #1 updated screening was overdue by two months.
Plan of Correction: The staff member was removed from the classroom and is no longer employed with the company. Staff are given 30 days notice of TB tests expiring and are required to complete them in that time frame.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On February 7, 2025, child # 1 (age 15 months) was observed crying, staff #1 was observed saying to a child #1 ?I can?t hold you all day", ?You need to walk? , ?You are delayed, the other
kids in here have you beat, and they aren?t even one yet?, in a harsh tone.
Plan of Correction: The staff member was removed from the classroom and is no longer employed with the company Our company policy for positive guidance and redirection will be reviewed with all staff at our march staff meeting.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

When a child is placed in a high chair, the protective belt shall be fastened securely .

Child #2 (age 11 months) and Child #3 (age 11 months), were both observed in high chairs with no safety belts engaged during the time of inspection.
Plan of Correction: Staff will be retrained on infant safety procedures for highchairs.
