The center shall ensure that areas and equipment of the center, inside, are maintained in a clean, safe and operable condition.
There was a small tear in the changing pad in classroom #1. Chipped and peeling paint were observed in classroom #2, #3, and #4.

Plan of Correction: Changing pad was removed the same day and disposed. Replacement ordered. Work orders have been entered for painting to be completed.
