During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 12/13/24. The on-site inspection began at 4:20pm and ended at 6:15pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and seven staff. The inspector reviewed five children?s records and five staff records. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/30/24. A POC submitted after this date will not appear on the public website.
Violations
4There is no documentation of a central registry search in the record of Staff #1 (employed for 2 and a half years). In the record of Staff #2 (employed for seven months), the documentation for the central registry search shows that the search results were not received until six months after the staff member's hire date.
The record for Staff #2 hasTB results that were submitted after the time of employment and not prior to coming into contact with children.
In the record for Staff #2 (employed seven months), the documentation for the Virginia Department of Education-sponsored orientation course show that the training was not completed within 90 calendar days of employment.
Documentation of orientation show that Staff #2 (employed for seven months) did not receive orientation training within the required timeframe. Orientation for the staff member was completed in the seventh month of being employed.
