Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on August 5, 2021 and concluded on August 6, 2021. The director was contacted by telephone to initiate the inspection. There were five children present and one staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed three staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, in one of three staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results prior to being hired.

Evidence: The director confirmed that there were no Fieldprint fingerprint results for Staff #3 (date of hire 7/1/2021).
Plan of Correction: We obtained fingerprint results from another agency and was unaware they would not be accepted. The staff member is no longer employed with our child care center at this time.
