On October 23, 2025, at approximately 2:30 p.m., Child #1 was observed standing in a crib. Staff #1 was seen twice pushing Child #1 on the chest with their hand, causing the child to fall back into the crib. Staff #1 also picked up a bottle from the floor and threw it into the crib toward Child #1. The center did not provide appropriate supervision.
