Inspector Notes

An unannounced inspection was initiated on 2-12-2026 and completed on 3-18-2026, in response to a complaint received by the licensing office on 2-11-2025 relating to allegations of 22.1 Religious Exempt. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does support the allegation; therefore, the complaint is determined to be valid.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3-26-2026. A POC submitted after this date will not appear on the public website.

On February 2, 2026, from approximately 7:55am until 8:05am Staff #1 was the only staff supervising 5 children and the youngest ,Child #3 was an infant. The required ratio for infants is one staff for every four infants.
Plan of Correction: We now have another assigned staff member to take the elementary children to the school so the staff working the waddler room does not have to leave the out of ratio.
