Inspector Notes

An unannounced inspection was initiated on 2-24-2026 and completed on 3-23-2026, in response to a complaint received by the licensing office on 2-17-2025 relating to allegations of 22.1 Religious Exempt. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does support the allegation; therefore, the complaint is determined to be valid.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3-30-2026.

Repeat Violation
On February 24, 2026, from approximately 7:40am until 8:01am Staff #1 was the only staff supervising 5 children including an infant (Child #1). The required ratio for infants is one staff for every four infants.
Plan of Correction: Through training, DSS explained I was following R.E subsidy rule which does not apply to us. So we now have a 2nd employee at 7am instead of 8am
