An unannounced, on-site code compliance inspection was initiated on 10/29/2024 and completed on 10/29/2024. The on-site inspection began at 9:39AM and ended at 11:57AM. The inspector reviewed compliance in the areas listed above. There were 28 children present with 8 staff. The inspector reviewed 3 children?s records and 3 staff records on site. This inspection included - document review, tour of the facility, interviews, and observations.
After review with the Licensing Administrator, the inspection documents were updated on 11/26/2024.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/13/2024. A POC submitted after this date will not appear on the public website.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #2, who had been employed for 6 months, did not have a health exam prior to employment.

The center shall establish and implement procedures for ensuring that all children in the center are in compliance regarding the immunizations of children against certain diseases. The record for Child #2, enrolled for 2 months, did not contain an immunization record.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #2, who had been employed for six months, did not have a completed national criminal background check.
