Inspector Notes

A code compliance inspection was conducted on October 2, 2023 from approximately 9:10 am to 10:00 am. There were thirty preschool age children present with six staff supervising. Additional staff were on site for administrative purposes. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 1 of 5 staff have annual documentation from a practicing physician or physician assistant finding the individual to be free from any disability which would prevent him or her from caring for children under their supervision.

Evidence: The record for staff #5 had a most recent staff health form dated 2/9/2022 which is more than a year old.
Plan of Correction: Staff member will see a physician or assistant to have a new staff health form completed.
