Inspector Notes

A monitoring renewal complaint inspection was conducted; there were 17 children present three staff supervising. Children were observed eating lunch and playing outside. .

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to get proof of identity and age for each child.
Evidence:
1. The inspector reviewed five children's records. Child #2 did did not have the required information.
Plan of Correction: The staff will get proof of ID and age from the parent.
