Inspector Notes

An unannounced Renewal Inspection was conducted on October 10, 20223 between the hours of 12:01pm and 1:51pm. There were 5 classrooms observed with a total of 44 children present, ranging in ages from 2 to 5 years with 7 staff supervising. Ratios were in compliance. The children were observed participating in story-time, coloring, playing with manipulatives, toileting routines hand-washing procedures, and other organized activities. A complete inspection of the physical plant, child and staff records, fire drill log, and emergency procedures were observed during this inspection. There was no medication on sight at the time of the inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to obtain documentation of a 5 year repeat check.
Evidence:
1. The record for Staff #1 did not have documentation of an updated Central Registry search results (expired 10/04/23).
Plan of Correction: We will ensure the search is submitted immediately when a new employee is hired.

Based on record review, the religious exempt child day center did not obtain the central registry finding by the 30th day of employment.
Evidence:
The record for Staff #4 (DOH 9/1/23) did not have the central registry results within 30 days of employment.
Plan of Correction: I will have the staff complete immediately.
