Inspector Notes
An unannounced monitoring inspection was conducted on 5/23/2022 from 10:30am to 1:16pm. At the time of entrance, 37 children were in care with 6 staff members present. Children were observed doing the following: playing on the playground, playing in centers, washing hands and eating lunch. A selection of staff and children records, the physical space, emergency supplies, evacuation drills and attendance records were reviewed. There was no medication on site. Violations
10Evidence:
1. Child #2 was under the age of two years old and had an immunization record dated 6/17/2021 and needed an update.
Evidence:
1. Staff #1 (date of hire 11/25/2019) completed 9.5 hours of training between 11/2020 and 11/2021.
2. Staff #2 (date of hire 4/20/2015) completed 4.5 hours of training between 4/2021 and 4/2022.
Evidence:
1. The last fire inspection was completed 1/2021..
Evidence:
1. In the rainbow classroom, paint was chipping under the window.
2. In the busy bees classroom, paint was chipping under the window over the A/C unit and the drywall was cracked.
3. In the souring scholars classroom, the bathroom wall directly across from the toilet had a long crack in the drywall on the wall over 1/2 the length of the wall along with chipped paint.
4. The outside playground door had rust on it.
5. Five plant pots were broken on the playground.
6. The water table on the playground was splintered.
7. The gate would not properly latch and the majority of two out of three sides of the fence was rusting. The fence structure was also bent on the left side and was pulling away from the fence supports.
8. The blue spinning structure had 8 cracks on the top of the equipment and one larger crack with a hole the size of quarter.
9. The black stairs on the slide equipment had multiple cracks and bent metal on two of the stairs with rust on both steps.
10. The stroller on the playground had standing water near the handle and seats.
Evidence:
1. In the rainbow classroom, there were 5 cleaning products under the sink in an unlocked cabinet. The safety lock was hanging from one of the cabinet handles and was not engaged.
Evidence:
1. The fence gate would not latch properly and had a chain and some type of lock to hold gate from swinging completely open. The opening was approximately 12 inches wide. The playground was located next to a busy parking lot and street. The fence was also a low chain fence with areas that needed repair.

1. The center's inspection dated 2/25/2021 was posted on the bulletin board along with the supplemental page. The supplemental page is considered confidential.
Evidence:
1. In the rainbow classroom, diapers were inside a lined trash can, but the foot petal was not working.
Evidence:
1. Child #1 (start date 8/05/2019) did not have previous child care and schools attended by the child and did not have documentation of child updates and confirmation of up to date information in the child's record as required by 8VAC20-780-420 E3. Staff #3 also stated Child #1 was no longer attending, but the last date of attendance was not recorded in the file.
2. Child #2 (start date 6/28/2021) did not have previous child care and schools attended by the child.
Evidence:
1. Child #1 (start date 8/05/2019) was listed on the center's food allergy list and had a diagnosed food allergy. The center had a FARE form for the child, but it was only signed by the parent and not filled out entirely. The physician's instructions were not given.
