Inspector Notes

A Subsidy Health and Safety inspection was conducted on 5/23/2022 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on record review, the center did not obtain the department's health and safety update course for each staff member.
Evidence:
1. Three staff records (Staff #1, Staff #2 and Staff #3-all completed annual update in March or April 2020) that were reviewed did not complete the Department's health and safety update course in 2021 or 2022 and had been more than 1 year since the last health and safety update course.
