Inspector Notes

A monitoring inspection was conducted today. There were a total of 44 children with 7 staff members present. Children were observed during lunch time, washing hands, playing with sand, and playing on the playground. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-4
Based on a review of staff records, they did not have sex offender checks for each state the employee has resided in for the past 5 years.
Evidence:
2 of 2 staff files checked did not contain a sex offended check for previous states lived in.
Plan of Correction: The Maryland forms were mailed. Although staff had VA Sex Offender when they live in MD they must have it as well.
Standard 8VAC20-780-350-A
Based on observation and staff interview, the maximum group size per teacher was not followed while children were in care.
Evidence:
Two teachers were observed with 24 children on the playground. The children were aged 3 1/2 - 4 years old. Teachers confirmed they each had 12 children.
Plan of Correction: There were enough staff present. 1 staff stayed with licensing inspector and should have gone outside. Will make sure ratio's are maintained.

Based on a review of child records, they did not obtain an annual update from the parents confirming correct information.
Evidence:
2 of 5 child files checked did not have an annual update. Child 1 had information dated 01/28/2021 and child 2 had information dated 8/31/2021.
Plan of Correction: Administration will make sure parents return yearly forms. Letter sent to parents.
