Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on October 21, 2021 and concluded on October 25, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 10 children present, ranging in ages from birth to 12 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five staff/household member records, one does not contain the required documentation of a child protective services central registry check within 30 days of turning 14 years old.

Evidence
The record for household member #1 (14 years old) does not contain documentation that a child protective services central registry check was completed.
Plan of Correction: A child protective central registry check form was mailed out on Monday, October 25, 2021. I will send my inspector the results when they are received.
