Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on 09/15/2021 and concluded on 09/23/2021. The director was contacted by telephone to initiate the inspection. There were 15 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of two staff's records, the center failed to ensure that all out of state background checks were completed for each state that the staff has lived in the past five years.

Evidence
1. Staff #2 is from a state that does not participate in the National Fingerprint File. Staff # 2 start was is 09/02/2021. The out of state criminal and sex offender has not been completed.
Plan of Correction: The administration office will submit the out of state backgrounds ASAP. In future all will be done before hire.
