Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/20/2025, as part of the licensure period. The on-site inspection began at 9:30 AM and ended at 11:12 AM. The inspectors reviewed compliance in the areas listed above. There were 13 children present and 3 staff. The inspectors reviewed 5 children?s records, and 3 staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standard or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 10/29/2025. A POC submitted after this date will not appear on the public website.

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.

The fall zone on the playground measured to be less than 6 feet when each piece of equipment required at least six feet of a fall zone.
Plan of Correction: Moved apparatus to fit fall zones. Removed extra apparati.
