Inspector Notes

An unannounced renewal inspection was conducted today between the hours of 11:30am-3:30pm. The physical plant, 4 staff records, 5 Agent records, 3 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed having lunch, participating in story time, and getting ready for nap time. Hand-washing procedures were also observed. There was an assortment of age appropriate materials for the children in care. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain the results of a search of the central registry and a sworn statement of affirmation every five years for 1 of 4 staff members reviewed.

Evidence:
1. The sworn statement in the record for Staff #1 was dated 09/18/2017.
2. The central registry check results in the record for Staff #1 was dated 08/14/2017.
Plan of Correction: A sworn statement of affirmation was completed during inspection. The director also submitted a central registry search during inspection.

Based on record review, required background information was not obtained before 30 days of assuming the role of agent for 5 of 6 Agents.

Evidence:
1. Agent #1 assumed responsibilities as an agent in October 2021. Results of a Central Registry search in the record for Agent #1 were dated 03/10/2023.
2. Agent #2 assumed responsibilities as an agent in October 2021. Results of a Central Registry search in the record for Agent #2 were dated 03/10/2023.
3. Agent #3 assumed responsibilities as an agent in October 2021. Results of a Central Registry search in the record for Agent #3 were dated 03/23/2023.
4. Agent #4 assumed responsibilities as an agent in October 2022. Results of a Central Registry search in the record for Agent #4 were dated 03/14/2023. The sworn statement of Affirmation in the record for Agent #4 was dated 03/04/2023. There were no fingerprint-based criminal history check results in the record for Agent #4.
5. Agent #5 assumed responsibilities as an agent in November 2022. The record for Agent #5 did not contain a sworn statement of affirmation, results of a central registry search, and fingerprint-based criminal history check results.
Plan of Correction: The Central Registry Search requests for Agents #1, 2, and 3 were submitted in February 2022; however a follow-up inquiry regarding the results was not completed. The center received the results in March 2023 and the documents were placed in the respective records. The background check results for Agents #4 and 5 were obtained and placed in the respective records.

Based on 4 staff records reviewed, 1 staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. Staff #2 began employment 5/24/2022. The record for Staff #2 did not contain documentation of completion of the Virginia Department of Education-sponsored orientation course.
Plan of Correction: The staff member is currently in the process of completing the training.
