Inspector Notes

An unannounced monitoring inspection was conducted on 3./22/2024 between the hours of 12:00pm-1:30pm. 29 children were observed directly supervised by 5 staff. The physical plant, 3 staff records, 3 children?s records, medications, and evacuation drills were inspected. Children were observed having snack and preparing for naptime. Areas of non-compliance are identified in the Violation Notice.

Standard 22.1-289.035-A
Based on record review, the center did not obtain a fingerprint-based criminal history record check every five years for 1 of 3 staff reviewed.

Evidence:
1. The fingerprint based criminal history record check results in the record for Staff #3 were dated 08/10/2023. The preceding fingerprint check results in the record were dated 06/14/2018, more than 5 years prior to the date of the most recent results.
Plan of Correction: The center will make sure all repeat background checks are obtained every 5 years.
Standard 8VAC20-780-240-A
Based on record review, 1 of 3 staff reviewed did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. Staff #2 began employment on 9/20/2023.
2. The record for Staff #2 did not contain documentation of the completion of Virginia Department of Education-sponsored orientation course
Plan of Correction: Staff #2 will be required to complete the preservice training by 4/1/2024.

Based on record review, 1 of 3 staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.

Evidence:
1. Staff #2 began employment on 9/20/2023.
2. The documentation of first aid and CPR training in the record for Staff #2 was dated 11/17/2023, more than 30 days from the first day of employment.
Plan of Correction: The center will ensure staff receive orientation training in first aid and CPR within 30 days of employment.

Based on record review, practice evacuation drills were not completed monthly.

1. The emergency drill log did not contain documentation of an emergency evacuation drill for the months of January 2024 and February 2024.
